Traumatisme psychique et troubles anxieux chez les enfants déplacés internes au nord du Togo
Psychological trauma and anxiety disorders among internally displaced children in northern Togo
Résumé : Le Togo a connu des situations critiques liées à des événements tels que les débordements de la crise du Sahel, les déplacements forcés de la population dont majoritairement les enfants, les afflux de réfugiés, les inondations, et d’autres catastrophes. La présente étude a pour objectif d’évaluer la prévalence des troubles anxieux chez les enfants déplacés internes victimes des attaques terroristes et d’analyser la relation entre le traumatisme psychique et les troubles anxieux. Il s’agit d’une étude prospective qui a concerné 350 enfants déplacés internes victimes des attaques terroristes dans la région des savanes au nord du Togo. Les techniques utilisées sont l’entretien semi-dirigé, l’observation, l’échelle Péritraumatic Distress Inventory – Child (PDI – C) et l’échelle Screen for Child Anxiety Related Disorders (SCARED). Le dépistage des troubles anxieux chez les enfants déplacés internes victimes des attaques terroristes montre une forte prévalence du trouble panique (90,4 %) et du trouble d’anxiété de séparation (92,8 %), suivis du refus scolaire anxieux (55,3 %), du trouble d’anxiété généralisée (51,2 %) et de la phobie sociale (39,9 %). Les résultats indiquent aussi qu’une augmentation du traumatisme psychique s’accompagne d’une intensification des troubles anxieux (R = 0,483), le traumatisme expliquant 23,3 % de leur variance (R² = 0,233). La prévalence des troubles liés à l’anxiété est extrêmement élevée au sein de la population des enfants déplacés internes victimes des attaques terroristes au nord du Togo.

Abstract : Togo has faced critical situations stemming from events such as the spillover of the Sahel crisis, forced population displacement (predominantly affecting children), refugee influxes, floods, and other disasters. This study aims to assess the prevalence of anxiety disorders among internally displaced children who were victims of terrorist attacks and to analyze the relationship between psychological trauma and anxiety disorders. It is a prospective study involving 350 internally displaced children who were victims of terrorist attacks in the Savanes region of northern Togo. The methods employed included semi-structured interviews, observation, the Peritraumatic Distress Inventory – Child (PDI-C) scale, and the Screen for Child Anxiety Related Disorders (SCARED) scale. Screening for anxiety disorders among these children revealed a high prevalence of panic disorder (90.4 %) and separation anxiety disorder (92.8%), followed by anxious school refusal (55.3 %), generalized anxiety disorder (51.2 %), and social phobia (39.9 %). The results also indicate that increased psychological trauma is associated with more severe anxiety disorders (R = 0.483), with trauma accounting for 23.3 % of the variance (R² = 0.233). The prevalence of anxiety-related disorders is extremely high among the population of internally displaced children who were victims of terrorist attacks in northern Togo.
